What is a Primary Care Network?

Primary Care Networks (PCN’s) are centrally funded by the NHS. A PCN is a group of GP practices working together with community, mental health, social care, pharmacy, hospital and voluntary services in a local area.

We take huge pride in our responsibility as a PCN, and employ our leading values in all we do.

Our aim as a PCN is to offer improved access and extend the range of services available to patients and help to integrate primary care with wider health and community services.
