Clacton Primary Care Network is proud to launch a new Proactive Care Service designed to support our population to live well and reduce avoidable hospital admissions.

Our growing Proactive Care Team includes a Proactive Lead, Proactive Nurse, Care Coordinators, Healthcare Assistant, Paramedic, Social Prescribers, Lower Limb Nurse, and Cancer Care Nurse. Together, they take a preventative, patient-centred approach—working closely with social care and council services through a multidisciplinary team (MDT) model. The team focuses on identifying and addressing health concerns early, with a particular emphasis on cardiovascular disease prevention through health education, screening, and expanding access to NHS Health Checks. A key priority is also supporting people living with frailty and long-term conditions, helping them to manage their health effectively and remain independent in their communities.
